OHIP (Ministry of Health)
MobilityFit Physiotherapy is a public community physiotherapy clinic. Ministry of Health funding can be available for persons 65+, 19 and under, following inpatient hospitalization, or OW/ODSP recipients.
A physician or nurse practitioner referral, valid Ontario Health Card and valid Ontario Drug Benefit (ODB) Card for OW/ODSP required.
Workplace Accident (Workplace Safety and Insurance Board)
MobilityFit is registered with the Workplace Safety and Insurance Board (WSIB) to offer physiotherapy to those who have been hurt at work and have a claim established. We complete assessments for different Programs of Care (POC) and work with WSIB adjudicators, case managers, and employers to provide appropriate treatment with the necessary paperwork to get you back to work safely.
Motor Vehicle Accidents Financial Services Commission of Ontario (FSCO)
We are a licensed clinic with Financial Services Commission of Ontario (FSCO) to treat Motor Vehicle Accident (MVA) injuries. Following an assessment, we create treatment plans on an individual basis for approval by insurers under the FSCO Guidelines.
Veterans Affairs Canada (DVA/VAC) for Veterans Care
We are registered with DVA/VAC. A doctor’s referral is required to start treatment through Veterans Affairs. We require the patient’s Veterans Affairs Card in order to seek approval from DVA/VAC for funding.
Extended Health Benefits
At MobilityFit, we are able to direct bill a number of extended health companies. These include Great-West Life, Sun Life, Green Shield, Blue Cross and some Manulife plans. The ability to bill these companies directly depends on the benefit package that is set up through your place of employment.
Some plans also require a doctor’s referral prior to your initial visit. It is important to contact your provider for details about your coverage. Plans may pay for a percentage of each visit, have a deductible or a co-pay option.
Patients who do not have coverage or whose plan does not allow direct billing are required to pay for their accounts as per our policy. Physiotherapy treatment is income tax deductible and is not subject to HST.
